Public Records Request – Scioto Valley Fire District

Ohio law provides that any person may request public records without providing a reason. This form is offered as a convenience to help ensure your request is clear and can be processed efficiently. You are not required to use this form to make a public records request.

Please describe the records you are requesting as specifically as possible (dates, incident numbers, addresses, types of reports, etc.). Clear and detailed requests help us locate records more efficiently.

Public Records Request Notice

The Scioto Valley Fire District processes public records requests in accordance with Ohio Revised Code §149.43. You are not required to provide your identity or the reason for your request.

The Fire District may ask for clarification if a request is overly broad or ambiguous. Records will be provided within a reasonable period of time. Certain information may be redacted or withheld if it is exempt from disclosure under Ohio law.

Fees:
Requests for copies of records may require payment of fees prior to release. The current fee schedule is as follows:

  • EMS Incident Reports: $5.00 per report
  • Fire Incident Reports: $5.00 per report
  • Additional charges may apply for large document requests, specialized media, postage, or other actual reproduction costs.

If fees apply to your request, you will be notified of the total cost before the records are produced.

Important – EMS Patient Care Reports:
EMS Patient Care Reports contain Protected Health Information (PHI) and are not released through public records requests. Requests for EMS Patient Care Reports must be submitted using the Scioto Valley Fire District Patient Request for Access to Protected Health Information (PHI) Form.

Accepted payment methods include cash, check, or other methods approved by the Scioto Valley Fire District. Records will not be released until all applicable fees are paid.
